Form IL-1040-V is the payment voucher used to submit any balance due on your Illinois individual income tax return (Form IL-1040). It accompanies checks or money orders mailed to the Illinois Department of Revenue. Taxpayers paying electronically do not need to submit this voucher.

Make your check or money order payable to “Illinois Department of Revenue.” Write your Social Security number and “2026 IL-1040” on your payment. Mail the completed voucher and payment to the address printed on the form or visit Illinois DOR Payment Options for electronic payment methods.

Example: Your Illinois total tax for 2026 is $4,200, and you had $3,500 withheld during the year. The difference is $700. You include a check for $700 with your IL-1040-V and mail it by April 15, 2026.

Filing Deadline

Payments must be postmarked by the due date of your Illinois individual income tax return (usually April 15). If the due date falls on a weekend or holiday, the next business day applies. Late payments may incur interest and penalties under 35 ILCS 5/804.
